-Phone Time-Out Coordinator: 0407 102 104

-Complete a Time-out Referral Form from a Coordinator.

-Fax the Referral Form to the Time-Out Office

-The Coordinator will pick up client who has chosen to be referred to the Time-Out House

-The Coordinator will take further instructions from referring person as to any issues which are appropriate, including recording any appointments that may be part of medical follow-up and arrangements for their exit from the house.

-Coordinator will transfer client to the Time-Out House
